Use this activity and workout tracker to submit for your Daily Workout rewards!
Our phones or our computer cannot be linked to devices. We do not have the resources to upgrade our phones or computer. We tried to link with the garmin vivoki last year and could not so we ordered the clearview pedometer. We each have one but our steps don't count now. What can we do to solve this?
I was just reading all the comments. I just bought and received in the mail yesterday a fitbit zip. A gal at my gym has one and uses go365 and told me to buy this device to track my steps to receive pts and cash value. So it sounds like this device is no longer acceptable. I hope Im not right. Is this an acceptable device? I just now finished setting up this device with fitbit and now hoping to set it up with go365. Please help.
Is the PDF form called medicare tracker something you are using instead of my fitbit zip now for 2019?
Or hopefully I can still use my fitbit zip!!
Fitbit Zip is still a compatible device.
How do I find an app on my phone that will link with the GO365 Humana Medicare program? My phone is a Google Pixel phone (Android). Also, how could I get one of the two Humana pedometers that are mentioned on this website?
Glad the option of filling in a form is available again. Too bad it cannot be done online. I get so much more exercise than I can record. So sorry the better rewards went away.
We are working on future enhancements to potentially allow for more online submissions.
Thank you for the feedback and keep up the great work!
You used to allow faxing of the monthly work out tracker. Do you still allow this, if so, what is the number.
Our fax number is 877-250-7814.
All I can say is that your Go365 program for Medicare recipients is Pathetic.
First you discontinue the Moves app, then you discontinue the use of the Humana Pedometer. For the past six months I have been forced to mail in my Activity Log and have repeatedly been told by your Customer Service agents that ...big things were on the way for 2019. AND THIS IS IT!!!!!!!! Not many Medicare aged subscribers can afford a $200 fitness to connect to your system, anyway the frequency at which you are discontinuing certain devices, why waste money on a device that will soon be obsolete. You certainly don't appear to be encouraging us senior to exercise regularly - wow a whole $5 for exercising 8 days a month. I walk 8-10,000 steps a day - good thing I do it for my health.
I thought your Go365 program was a great idea, but it has become an insult to Medicare recipients. I doubt if I will participate, and I will definitely take this into account when I consider my choices for healhtcare next year
Is there a way to text a photo of the Workout Tracker?
How do I manually give you my workout activity?
I keep looking for list of electronic means, but still not there.
Expense is a concern.
Can I email a form to you with activity?
I do not have a fitness tracker and did not know a program exists. Is there a paper form that can be filled out. I attend Senior Aerobics classes twice a week and the work out lasts for 1 hour. I also try to walk 2 miles a day for 3 or 4 days a week and I ride a stationary bike 15 miles a night. How can I get credit for these activities. I also eat healthy and have lost 24 pounds since July. My BMI is now normal. Thank you.
I have downloaded and printed out the Go365 By Humana Workout Tracker. I take it that the Member ID# is the number that is on my Humana Health Card. Is this correct? Just want to be sure and fill it out correctly as I will do this monthly. Thank you for answering my previous questions. I appreciate it and this is good information. Does Planet Fitness honor the Silver Sneakers Program. I was under the impression that only the YMCA honored the program and I know the YMCA here in Suffolk, VA 23434 where I live does not honor the program, but a YMCA 30 minutes or more from here does and I am not traveling that far. But we do have Planet Fitness here and I already belong there if they honor Silver Sneakers I would like to know. I take the Aerobics Class at a recreation center and pay $12 for 6 weeks which is a real bargain and I love it.
Yes, that would be your member ID. (Typically starts with an "H")
Silver Sneakers partners with all different gyms.
You can check eligibility here:
You can also earn rewards through participating in a community or social event (aerobics class would be part of this!)
What kind of activities are eligible for rewards?
While we do not have a way to text the form, you can fax to 877-250-7814
Here is the tracker:

I agree. I used to have Go2 pedometer. all I had to do was plug it in each day, and it recorded my steps.
They need to make things easier for seniors and not harder. we should be able to fill in Step coupons on line.
How about taking a pic of workout coupon and texting it to Go 365?
I agree with you, Howard. Like you, I walk that amount of steps each day, too. The way this is all set up amounts to a big waste.
We do not have a way to receive text messages. You can fax to 877-250-7814.
Last year I was able to upload through my computer my monthly activity. Then I was emailing them to a representative. Can I do either of these to submit my activity without a tracker? I'm trying to figure out how to submit my January 2019 activity. I do not have a fax machine but I am able to take a picture of my filled out form and upload it to my computer. Thank you.
You can report these via mailing in your tracker, calling us with the information over the phone, or by chatting with one of our representatives.
Regarding rewards for go365, your web page says, "Qualifying workouts include SilverSneakers® or walking a minimum of 500 steps a day."
I am not sure what "qualifying workouts/SilverSneaders means. I do not walk, but ride a stationary bike. Does that not count?
This new workout qualifications and reporting process is a piece of junk. I can get 500 steps every day just getting up, making coffee, and going out for the newspaper. Eight days a month is a ridiculously small amount of exercise and not what doctors recommend.
