Dr. Zelenka submitted and faxed a biometric form for me on 10/9/18. I have not received full credit of the 4000 points . I called in and the person I spoke to could not tell me if you received her fax and indicated that they were not , as of 10 /12 processing 10/9 received forms . Please advise me what is going on with my Biometric Form , I can have her re- fax if necessary.
