After I sent the first link when I went back to send the bone density test it wouldn't go back any further than 12/4/2019. That is the date of the first test.
Are you attempting to self-submit activities on your Go365 account? If so, you should be able to manually enter the date in XX/XX/XXXX format.
If you continue to experience issues, please contact Go365 customer service for additional assistance. You can also call 1-866-677-0999 to verify your rewards balance, and redeem your rewards. Please keep in mind that this needs to be completed prior to 12/31/19, or rewards will be forfeited.
