looking for form to put in my socialization
Here is the form to submit Social Activity completion: Go365 Medicare Social Activity Form
The form is located under the "Forms" drop-down at the top of the page.
You can also submit a social activity directly on your Go365.com account. To do so, you start by clicking the Social Activities tab on your Go365 Dashboard. Once you see the Social and lifestyle activities page appear, scroll down until you see "Let us know once you've complete one."
Here is where you choose the activity type, enter a date, location, and brief description of what you did. You then click "Submit" and your rewards will be applied to your account.
You can be awarded for up to four social/community activities per year. Please let me know if you have any additional questions!
