Looks like you only reward tests taken before the middle of December since it looks like documentation must be received by Dec. 15th.
Thank you for reaching out to us! All activities completed between 1/1 and 12/31 are eligible for rewards. However, the reward needs to be applied, and the member needs to redeem their rewards, prior to 12/31.
Proof of prevention activities are typically received via claims, after the provider bills Humana. However, how quickly we receive that information depends on how quickly the provider's office sends that to us. The member can self-submit proof, but we do need to allow for processing time.
If you are having a screening completed post 12/15, it is best to obtain proof of the activity from your provider and then contact Go365 Customer Service directly for assistance (ASAP, but definitely prior to 12/31).
