Were you signed into your Go365.com account? This website (Go365 Medicare Community) is for members to ask questions and collaborate with others members.
You can access your Go365.com account by signing into your Humana.com account, and then clicking on the Go365 tab. You will then need to click on "Go to Go365." It will tell you that you are leaving Humana, and that is correct. If you haven't already, you will also need to complete the 2-step verification process, prior to accessing your Go365 account.

If a qualifying activity is not awarded via claims (maybe your provider did not individually bill for each item completed, if you did not use your Humana insurance, paid out of pocket, or the service was free), you can upload proof of activity completion on your Go365.com account. If you had the qualifying activity, you can sign into Go365, click "Preventive Care," and then "View details" under the applicable activity. If you scroll to the bottom of the details page, you can select the date of service and upload sufficient proof of the activity completion.
