I received my flu shot 09/09/20. How do I receive my reward?

If a qualifying activity is not awarded via claims (maybe your provider did not individually bill for each item completed, if you did not use your Humana insurance, paid out of pocket, or the service was free), you can upload proof of activity completion on your Go365.com account. If you had the qualifying activity, you can sign into Go365, click "Preventive Care," and then "View details" under the applicable activity. If you scroll to the bottom of the details page, you can select the date of service and upload sufficient proof of the activity completion.
